Derek Jeter’s, 40, last home game in Yankee Stadium on Sept. 25 was everything we could have wanted and more. The 20-year veteran scored the game-winning run in his last home game, making the final score 6-5. He hugged his gorgeous model girlfriend Hannah Davis, 24, on the field after the game. What a special moment. Hannah, being a devoted girlfriend, was in the stands throughout the game supporting her man.

After the emotional game, Hannah, his mother and nephew were on the field ready to greet their man. Derek and Hannah shared a sweet hug on the field in the midst of thousands of fans sitting in Yankee Stadium.
Earlier, Derek was kneeling on his shortstop position when fellow player Jorge Posada pointed out that Derek’s girlfriend, mother, father and nephew were all sitting behind home plate in order to surprise the 20-year veteran.
Derek ran over behind the home plate and put his hands through the netting to reach out to Hannah and his mother.

Derek and Hannah held hands through the netting as the Yankees player said a few words to his mother. So cute!
Hannah was beaming from ear to ear as she showed a little PDA with her man, who was undoubtedly the man of the night.
Yankee Stadium was full of emotion on Sept. 25 as Derek took his final bow. This humble and incredibly talented man will be missed in the game of baseball.
